Parents must register their child/children at the Alliance for Women & Children located at 1350 North 10th Street during regular business hours, 8:30 am – 5:00 pm. The registration fee is $25 per child with a maximum of $50 for multiple children.

1. Parent’s home address(es) and phone number(s)
2. Employer phone numbers
3. Two (2) emergency contact names and phone numbers
4. Doctor’s name, address, and phone number
